Client Service Agreement + Consent
Combat Sports Consulting

1. Overview

This agreement outlines the terms of service between Combat Sports Consulting (“Provider”) and the client (“Client”) for online injury assessment and rehabilitation services.

By engaging in services, the Client agrees to the terms below.

2. Nature of Services

Services provided may include:
- Online injury assessment and guidance
- Exercise-based rehabilitation programs
- Load management and return-to-training advice
- General education relating to injury and performance

Services are delivered remotely (online only) and are based on the information provided by the Client.

3. Not a Substitute for Medical Care

The Client acknowledges that:
- Services do not replace medical diagnosis or treatment
- The Provider does not prescribe medication
- The Client should seek medical attention for serious or worsening symptoms

4. Client Responsibilities

The Client agrees to:
- Provide accurate and honest information
- Follow guidance within their tolerance and capacity
- Stop exercises if pain significantly worsens
- Seek further care if symptoms deteriorate

5. Payment Terms

Payment is partially required upfront to secure services.
Remaining balance (if applicable) is due as agreed.
Services may not be delivered until initial payment is received.

All payments are non-refundable once services have commenced.

6. Cancellations & Rescheduling

Clients are encouraged to provide reasonable notice if rescheduling.
Missed sessions may be forfeited at the Provider’s discretion.

7. Limitation of Liability

To the extent permitted by law:
- The Provider is not liable for any injury, aggravation, or outcome resulting from participation in recommended activities
- The Client acknowledges that all physical activity carries inherent risk

8. Informed Consent

The Client understands and agrees that:
- Exercise-based rehabilitation involves physical exertion and potential risk
- Outcomes cannot be guaranteed
- Recommendations are based on best available information but may vary between individuals

By proceeding, the Client gives informed consent to participate in services.

9. Privacy

Client information (including name, contact details, and injury information) will be:
- Used solely for service delivery
- Stored securely
- Not shared without consent, unless required by law

10. Acceptance

By engaging in services, the Client acknowledges they have:
- Read and understood this agreement
- Had the opportunity to ask questions
- Agreed to proceed voluntarily
